Camp Nicolet Health Forms

There are three health forms that must be completed before your camper can attend camp. All three forms are PDF files. You will need the Adobe Acrobat Reader to view the forms. If don't already have the Reader, click here to download it.

Once you have completely displayed these forms, print one copy of each, fill them out, and either mail or fax them to the camp office.
